Employees at the California Public Utilities Commission misclassify almost 40 percent of complaints they receive from the public, leaving the agency ill-equipped to properly address consumer grievances, a report released Thursday concludes.

Auditors also found serious weaknesses in the commission’s information-technology systems and documentation, leaving it vulnerable to computer security threats. Also, the agency has no incident-response plan in case of a cyber attack, the report said.
